The following materials must be submitted to the Internship Director postmarked by February 15, 2008:

1. ADA Dietetic Intern Application Form version 2005: (click for the link)completed, dated and signed and including a telephone number where you can be reached on April 22, 2008 between the hours of 8:00 a.m. and 3:00 p.m., Pacific Time.

2
Official transcripts: 1 copy from each university or college attended after high school. Grade report for first term of senior year (if not included on transcripts). A legible carbon or written copy is acceptable if authenticated by Didactic Program in Dietetics Director.
|
|
3. A copy of your DPD check list or advisement form. This is a list from your university that states which classes meet DPD requirements.
|
4. Typed letter of application stating:
- Why you chose this particular Dietetic Internship Program
- What your Long- and short- term career goals are and how you feel an internship with the VA will help you achieve these goals
- What unique qualities you can bring to the VA
- If and how you have financed your education using these unique qualities
|

5. Declaration of Intent to Complete Degree and Academic Requirements or Verification Statement for completion of Didactic Program requirements. Either statement must include an original signature from the Didactic Program in Dietetics Director.
|
|
6. Three letters of recommendation:
- one from your Didactic Program Director
- one from an employer, or supervisor of paid or volunteer work
- one from a professor of Dietetics or related courses
Ask the professors to identify the course(s) in which they were your instructors. Use the ADA DI Recommendation Form version 2004 (click for the link)

| 7. Recent Education Form (click for the link)
ONLY if dietetics coursework was completed 4 or more years prior to the application deadline

| 8. Completed Supplemental Information Form (click for the link).

| 9. If you wish to be notified that your application was received, please enclose a self-addressed, stamped postcard.

| Combined Internship - MPH Track
If you are applying to BOTH the Non-Degree and Combined Masters track, use the Combined Internship guidelines below for your application contents
|

|
1. You must submit the same application materials for the non-degree
program, steps 1 – 9 above, to the Dietetic Internship Director by
February 15, 2008.

|
2. Applicants must complete the UCLA Application for Graduate Admission and the School of Public Health Supplemental Application online including all supporting documentation. These application materials can be accessed by visiting http://www.ph.ucla.edu/ *
|
|
|
3. Non-refundable application fee of $60. A credit card number is accepted with the online application, or send a check or money order payable to Regents of the University of California to the Student Service Office.
|
|
|
4. Official GRE scores must be sent to UCLA by February 15, 2008. Please use the Institution and Department codes listed on the online graduate application when requesting that test scores be forwarded.

|
|
The application deadlines stated in the UCLA "Application for Graduate Admission" do not apply to application for the Combined VA Dietetic Internship - MPH Program. The deadline for applications to the Combined program and UCLA GRADUATE MPH PROGRAM is FEBRUARY 15, 2008

Combined Internship - MS Track If you are applying to BOTH the Non-Degree and Combined Masters track, use the Combined Internship guidelines for your application contents
|

1. You must submit the same application materials for the non-degree program, steps 1 – 9 above, to the Dietetic Internship Director by February 15, 2008
|

2. You must submit a Graduate/Post-Baccalaureate Admissions Application, accompanied by the $55 nonrefundable application fee. You must also submit official transcripts of all college coursework to Enrollment Services/Admissions, CSU Long Beach, 1250 Bellflower Blvd., Long Beach, CA 90840. Check with the specific program for additional documents and requirements.
It is strongly encouraged that you apply online by visiting CSUMentor.* If you don't have access to the Internet, you may obtain an admissions application from CSULB or another CSU campus.
3. Students must concurrently submit applications to the University Admissions and Records office as above and to the Department of Family and Consumer Sciences. Application instructions can be obtained from http://www.csulb.edu/colleges/chhs/departments/fcs/docs/procedures.pdf*

The application deadlines stated in the CSULB FCS Grad Website do not apply to application for the Combined VA Dietetic Internship - MS Program. The deadline for applications to the Combined program and CSULB GRADUATE MS PROGRAM is FEBRUARY 1, 2008 NOT the FEBRUARY 15 deadline.
