PERC Coordinators: Our Patient Centered Care Story
From left to right: Fiona Hwang (LAACC), Laura Camberos (WLA) & Rachel Rosen (SACC).
The Patient Education Resource Centers’ (PERC) goal is to educate veterans about health to give them the information and tools necessary to make changes towards a healthier life.
Three PERCs are located within GLA and are each run by, PERC coordinators. Our patient centered care services include providing education to veterans about conditions and diseases, medications, and healthy habits through health classes, health events, and multimedia.
We have served over 12,700 people for the 2010 Fiscal Year through classes such as the Smoking Cessation Clinics and Diabetes Education Classes, and health events such as, Wear Red Day and March Towards My HealtheVet.
Many Veterans do appreciate the services that we provide and have said the following about us: “PERC serves us wonderfully,” “The staff is ‘A’ ok,” “Doing a good job,” and “I get here and there is always the best staff to help me.” Education is a great tool to help enhance health and well-being and we, the PERC coordinators, are here to provide this patient centered care service to the Veterans.
