General Internal Medicine

GOAL

Supervised clinical block rotations in geriatric medicine across the continuum of care.

OBJECTIVES

Varies by healthcare setting.

Nursing Home (Sepulveda and West Los Angeles): Institutional care of complicated frail geriatric patients under supervision of GRECC attending physician.
  • Provide primary care to a panel of nursing home residents.
  • Deliver medical care within a team approach, including nurse practitioners, registered nurses, geriatric pharmacists, social workers, geropsychiatrists, kinesio and occupational therapists, wound care nurse specialist and other healthcare professionals.
  • Develop and present didactic lectures on geriatric topics.
Geriatric Medicine Clinic (Sepulveda and West Los Angeles): Care of medically complex patients with geriatric syndromes in an ambulatory care setting under the supervision of GRECC attending physician.
  • Training and experience in performing geriatric screening and assessment in patients referred to the Geriatric Medicine Clinic for comprehensive geriatric assessment and/or primary care.
Geriatric Evaluation and Management Unit (West Los Angeles): Subacute level of care for medically complex patients with geriatric syndromes who are at risk of losing independent community function, supervised by the GRECC attending physician.
  • Inpatient training in the interdisciplinary evaluation and management.
  • Direct monitoring of treatment and therapy to enable the implementation of individualized patient care plans in the context of a rehabilitation setting that is supported by strong neuropsychosocial assessment.
  • Participate in interdisciplinary team and family meetings, which are powerful and effective tools to develop, communicate and carry out specific care plans.
